The University of the Philippines Manila School of Health Sciences, Extension Campus, Tarlac Province is still accepting applicants for Academic Year 2021-2022.

To inquire the availability of scholarship slots in your Municipality, please visit your respective LGUs or the Mayor’s office for information.

You may also inquire scholarship slots at the UP SHS Tarlac Building in San Isidro Tarlac City for inquiries, our core faculty will be there to assist you Monday to Friday from 9 am – 5 pm.

Please see the Recruitment and Admission Primer that was posted below for the COMPLETE list of requirements. However, due to the COVID19 situation, we will consider an applicant’s initial submission of the following requirements:

1. Brgy Resolution endorsing the scholar
2. Brgy Profile
3. Signed RSA with SB resolution authorizing Mayor to enter into contract supported by a MOA
4. BIR ITR

We would like to stress on the commitment of future scholars to SUBMIT/COMPLY ALL other requirements AFTER passing both the assessment of documents and interview for your acceptance in our school. And if other requirements are readily available kindly include these upon submission.

All applicants are requested to email scanned copy of requirements at shstarlac.upm@up.edu.ph. Original copies should be mailed or submitted at the 2nd floor Community-Based Monitoring System (CBMS) Office at the Maria Clara Chest Center Bldg., San Vicente Tarlac City or at the UP SHS Building in San Isidro Tarlac City.
